Income
Name, address and phone number
of employer(s) for 2 years
Position, salary and term of employment. 
Person to contact.
Most recent 30 day's paystubs showing YTD earnings. 
Relocation agreement 
Award letters (retirement, pension, SSI, etc)
Evidence of receipt of child support or alimony payments (12 month history)

Residence Information
Address(s) for the past 2 years
If renting: landlords name, address and phone number If you currently own a home: name, address and account number of mortgage holder and loan balance 
Sales contract, closing statement, or lease agreement on your present home
If you have sold a home in the past 2 years, a copy of the closing statement or a "HUD 1" settlement sheet

Banking Information
Bank or investment account statements for the last 3 months for all accounts (all pages)
Address of bank or investment firm if not on statement
401-K, IRA and company thrift plan statements covering the most recent 3 months available, including documentation of any loans (i.e. quarterly, semiannual or annual statement)
Copy of notes receivable
Documentation of any large deposits other than payroll deposits
Gift Funds
Gift letter: we will supply to you.
Copy of gift check
Proof of receipt of funds
Proof of donor's ability to provide funds

If You Are Refinancing
Legal description (copy of deed)
Year end mortgage statement to show taxes and insurance
Copy of closing statement from purchase (FHA or VA loans only)
Copy of homeowner's insurance policy

 

If Bonus or Commission Income Is More Than 25% of Gross Income
Signed copies of last two year's personal tax returns with all schedules attached

Self-Employed or Sole Proprietor
YTD profit and loss statement
corporate or partnership returns with all schedules for the past 2 years Personal tax returns with all schedules for the past 2 years

Monthly Financial Obligations
Name and address of creditors
Account numbers, balances & monthly payments
copies of most recent statements
Evidence of payment of child support and /or alimony

If Currently Active
Copy of orders if being transferred
Statement of service
Current Leave & Earnings Statement (LES)
Documentation for any advance pays

 

Partnership Income
Complete K-1's

Problem Credit or Bankruptcy
Letter of explanation of circumstances
Bankruptcy discharge and debt statement

Fully executed sales contract with original signatures for new purchases (all addendum's to be included) Divorce decree (final decree and recorded separation/support agreement)

Veterans Administration Loans
